CLASSROOM TRAINING AND TEST DATES FOR

Through November, 2009
□ November 10-12, 2009 20 hour training for Category I applicants &
Section B exam
□ November 10-12, 2009 30 hour training for Category II applicants &
Section B exam
CLASSROOM TRAINING AND TEST DATES FOR ASSISTED LIVING
ADMINISTRATOR APPLICANTS
Through November, 2010
Please check the dates you wish to register for to sit for the
examinations and take the required training.
Please remember the first step is to take and pass the Section A
exam on the Alabama Department of Public Health’s Rules and
Regulations for Assisted Living Facilities or Specialty Care
Assisted Living Facilities, and then you will attend the Classroom
Training program and take the Section B exam.
If you are applying for a Category II license, you must register to
attend the 8 hour dementia training prior to taking Section A of the
Category II examination.
Please return this form with your completed application. At the
current time, testing and training will take place in Montgomery.
You will be notified by mail of the exact time and location of your
testing and training.
